a) that to the best of my knowledge, the information provided in this application is true and complete;

b) that I propose to be a full-time student at the institution named for the period stated;

c) that I am an Alberta resident, as defined by the Alberta Heritage Scholarship Act;

d) that I agree to allow my name, accomplishments and study plans to be released publicly if I receive this award;

e) that I grant permission for the Society to request and receive information pertaining to my post-second enrolment status and academic progress; and

f) that I authorize the release and exchange of my personal information for use in research and statistical analysis or in program evaluation.

Also, if you are a first year student at a post-secondary school, in addition to the post-secondary school first semester transcript, please include your high school transcript.
